Terms and Conditions
A. Memberships starts upon the full payment of the annual premium. He can avail of his PLAN’S benefits on the same month he enrolled if he has paid his premium on or before the 5th day of the month he paid his annual premium.
B. He will be issued a Health Plan Card (CARD). There is a two weeks wait for the receipt of his CARD. He can, however, avail of the benefits by presenting the Official Receipt (O.R) and a picture ID to the reception personnel.
C. The member can only avail of the laboratory and diagnostic services referred by the clinic’s physicians.
D. No refund of membership premium will be made to the member once he has availed of any of the benefits of his PLAN. This application is subject to acceptance by the management of Kaiser Medical Center. The acceptance and approval of this application are based on the statements by the member in his application.
E. Kaiser Medical Center has the option to terminate the membership of the applicant if the information in the application is false or the use of benefits are deemed to be inappropriate. The of the benefits availed upon will be deducted from the adjusted premium. Thus, if the membership is terminated on the second month of membership, Kaiser will refund the premium for ten (10) months less the retail price of the service(s) availed of.
