i don't have any valid id's left but my sss. i need another one and is thinking of just getting a philhealth id card. i have that old one without the picture. who has tried getting a philhealth id card? what are the requirements? what branch? i'm no longer connected to any private firms but is planning to go on voluntary contribution if allowed.

Ala ko kabaw sa requirements....pero sa akong nahibaw.an ang ilang office sa Cebu is murag kanang naa ubos sa Golden Peak Hotel diha sa may Escario....atbang ni siya sa Old SSS nga office.....

you can go there anytime within office hours nila.. when I got mine kay nagkuha ra ko MDR ato and at the same time I told them I'd like to get the ID tapos gihatag nila sa ako, it will have your name and your Philhealth Number printed tapos butangan na lang nimo picture and then you can have it laminated. TIN ID from the BIR is also a valid ID man pud.

i think i already had a philhealth ID pero wala nako malaminate and nawala na.

mao pud ni ako problema kay 1 ra jud akong valid ID which is voters. kelangan nako mokuha og 2nd ID to get my quit claim from my previous company. kato niadto ko philhealth taas man kaayo to linya oi.

naa na plastic id sa philhealth ag iphilhealthy card at the same time discount card sd sa mga partner merchant nga mga pharmacy.. pero d gihapon xa considered as id ky wala xa pic..
actually i have it na. last november ko nakakuha mobile distribution daw to nila... sa whole region 7 BIR offices ila schedule. twice na sd nko nagamit as discount sa watsons ug rosepharmacy.
it cost only 100 for the existing members 150 yata ag sa new.
if like ninyo ag plastic nga card sa philHealth ag tig-issue sa card naa sd ga-office kilid sa Golden Peak which is near Philhealth office ra sd kanang jeepney stop sa 04L.

bag.o lang ko kuha akong wife ani TS. dali ra man daw. may company ra siya gi present and nakita ra iyang MDR. adto lang jud daw before 4pm although until 5pm ilang business hours i think.

valid id's lang ts?how about driver's license and passport. valid pa na sa tanang valid. kuha lang gud time will come mangihahanglan ra ka ani.
