With your pc directly connected to the modem, I assume you will have a public ip address, which is seen in the internet. You may install applications like RemotePC, PcAnywhere or just enable the remote desktop application that comes with your OS.
Next, you need to know what is the ip address your pc receives. Open Command Prompt then type, ipconfig/all. Get the ip address and take note of it.
Once in your office, try opening the Remote Desktop application on your pc and type the IP address of your Home pc to the text box and voila, you can connect to your home pc.
Just remember:
It must be allowed by your Office firewall and your PC Firewall to connect these kinds of applications.

Haven't you try logmein? Anyway if you have any existing firewall (Juniper for example) part of the package is what we called netscreen remote, although its a bit complicated in terms of configurations since it requires tunelling, policy and gateways. I'm using this one for our Manila Office for them to connect remotely thru secure tunnel here in Cebu. But try logmein its secured in anyway since its encrypted also. You install logmein free in your home and browse the website www.logmein.com in your office and provide the user credentials back there in your home p.c.And you are now connected instantly.

modem to modem ( local mode )
dsl to dsl ( internet mode )
router to pc - vice versa ( depend the features from a router )
operating system:
windows any version
linux any version
mac OS any version
