Parental Advice for Marriage License Appilication

Hello!
We need help!
My fiance is 25yrs old and we're getting married this year. One of the requirements for Marriage License is a Parental Advice for age 22-25...does anyone know's or have a sample template for this? thanks so so much

actually mam ,there's a form already sa "marriage" office.. all u have to do is bring ur parent (if ikaw ang below 25) then let ur parent sign the form and submit it to the office..

last year lang, nakaagi ko ana.. hehehe

thanks for the info pwede ba magget lang ng form? nasa ozamis kc ang parents ng fiance ko...hehe

i think, what wud they suggest is to let ur fiance's parent get the "parental advice" form in ozamis and send it here..

yan din ung ginawa ng parent ng wife ko, coz they're in bohol..

pero siguro, mas ok if ul ask nalang in the office, just to make sure.
pero sigurado ako na ung form eh manggagaling sa "marriage" dept/office..

kasi gumawa din ako dati ng form, natawa ung officer, may form naman daw kasi sila.. hehehe
