What is your webhost? Does it support cpanel or plesk?
IP addresses or domains can easily be blocked using cpanel. Just click "Ip deny Manager".

For tracking the IP address of visitors, you could view this on your raw access logs.

If you have full control of your web server machine and running in linux.
Use iptables to block incoming access to port 80. Access log is found in /var/log/apache(2)/access.log.
