Chong Hua Hospital Fellowship Training Application Form (to be filled-up at CME office only)
Application letter addressed to Medical Director through the Assistant Medical Director for Continuing Medical Education (pls. see below for the correct format)
Application Form / Resume
Picture 2x2 (2 pieces) should be less than 6 months, with white background
Letters of recommendation addressed to the Assistant Medical Director for Continuing Medical Education from the following:
Department Chairman where you completed Residency Training
Program Director for Residency Training
Program Director for Post Graduate Internship
The LETTER OF RECOMMENDATION should be in the OFFICIAL LETTERHEAD of the recommending individual and should be submitted in SEALED ENVELOPE.
(Photocopy of the following documents)

Transcript of Records from where you obtained medical school degree
School Ranking should be issued by Dean or Registrar of Medical School
Medical School Diploma
Certificates of Training
Certificate of Post Graduate Internship
Certificate of Residency Training

gikan ko ganiha sa PNA cebu kay ngrenew sa ID.. and naa ko naka storya ganiha didto na ngapply pud siya sa Chong hua.. last december 2014 pa sya ngpass and take exam pero last week pa sya na-update na pakuhaon na siya sa final requiremnt for employment na.. grabeh ana kadugay maghulat diay.. hehe

I went to mactan doctors hosp para mu submit ug application.. wala man lamg dawata.. dli daw ko pwede kay dli from cdu uc swu or usc ba to.. waah sayang au ang money nagasto sa reqts.. kinsa naa list sa reqts sa chong hua or uc med? Try nlng ko pass para d masayang ang docs.. slamat..
