Never wave around the knife, spoon and/or spoon while talking. (ie like an orchestra conductor!)

Always outside in. Once you're seated on your table place the napkin on your lap, not your collar. When bread is offered take the bread and take some butter. Do not place the butter directly on your bread place it on your bread plate first. (you'll know a butter knife because its usually small and have a blunt edge). Bread plate is usually placed at the top of your forks.
The forks are on the left side. The outer most is the salad fork, then fork for the entree (This is usually the biggest fork), then the dessert fork.
The spoons and knifes are on the right side. The outermost is the soup spoon (small, most times its rounded shaped). Then a teaspoon used to stir coffee. Then your knife.
Sometimes dessert forks and teaspoons are at the top of your plate.
Water glass/goblet is the inner most (just above the knife). Any goblet to the right is for wine.
Place any used utensils on the plates you used it from. Do not place dirty utensils on the table cloth or placemats. If the plate came with a saucer place the utensil on the saucer.
****DO NOT talk/text on your phone while at the table. If you really need to use your phone, excuse yourself. Silence your phone while at the table.
    Always thank your server.

Always thank your server. => the best.

Also, kung gamay na lang ang soup sa imo bowl, i-tilt man siguro na siya padulong center sa table. Hold the side of bowl na naa directly in-front sa imo-a then lift it slightly. Then, use the soup spoon para pagkuha sa remaining soup without making any sound. Dili gunitan ang both sides sa bowl dala higop.

i think its better to hold the glass of water with your pinkie sa lobot sa baso para likay slip sa baso.

Pet peeve nako ang mag make ug noise when chewing food. When I was a student, this fellow intern from another school did just that, and he was from an extremely well-off family. Minus gyud kaayo. Keep elbows of the table as well. And basic gyud ang not talking when one's mouth is full.
