Logic = boolean algebra, it's a variant of elementary algebra, still part of Mathematics.
When we hire new programmers in our company, we run them through a set of mathematical exams first, before we even test their programming skills. The reason - our company (Prop trading firm) is into Quant - we crunch financial data from different stock exchanges in asia - ASX, SGX, SMX, etc .. looking for patterns, predicting the next direction for trade. It requires a lot of math (integral and differential equations). In fact, we prototype our algorithms first in Matlab, then transition the code to C++ only after running a bunch of simulations.
Mathematics is solving problems using its formula to come out a result. Say you are given a mathematical problem and you are going to solve it for that problem while in programming you are going to construct a statement given the formula thru syntaxes. In short in programming you are designing a system that solve multiple problem in mathematics at once.
It depends on the requirements, sometimes in programming or most of the time involves computation like my accounting system, there are computations involve but not necessary you are good in mathematics.

you dont need to be a math wizard to be and effective programmer you dont need math to do a system request, you dont need math to check whats the weather, you dont need math to properly display a website.... math are needed only on certain places not everything unless your doing a accounting application... where you deal with lots of numbers
