Cure & Treatment for Palmar hyperhidrosis or excessive hand sweating

Palmar hyperhidrosis or excessive hand sweating is the most common and troublesome form of hyperhidrosis. People with palmar hyperhidrosis have cold and clammy hands that sweat excessively. Hand sweating can be initiated by emotional distress, nervousness, stressful situations and anxiety. Excessive sweating could also be seen with no stimulus.

Sweaty hands may also be associated with excessive sweating syndrome consisting of palmar hyperhidrosis, sweaty armpits, sweaty face or sweaty feet. However the most common combination is sweaty palms and sweaty feet. Symptoms usually start in early teens and continue through entire life.
Palmar hyperhidrosis affects every aspect of one’s life. People become constantly aware of this problem and try to hide it from others. Affected people cannot control their hand sweating, which can also be aggravated by emotional distress. Many patients feel a tingling sensation and think their skin pores are opening prior to the onset of sweating. These individuals may experience some or all of the following:

Constantly trying to wipe their hands

    Avoid shaking hands

Have difficulty with typing, writing, and handling papers

Difficulty putting on make-up and wearing clothing

Uncomfortable initiating intimate relationships and socially withdrawn

Preoccupied, anxious, and have poor concentration

Difficulty handling money or playing musical instruments

    Difficulty with getting a manicure

A firm solid handshake often makes an important first impression. For those who suffer from palmar hyperhidrosis, that first handshake can be a dreaded event leading to avoidance of many potentially beneficial professional and social encounters. Many people may interpret a cold, wet, clammy handshake as a sign of insecurity, which in a job interview can translate, to a lack of self-confidence. Consequently this can hinder success and achievement in the professional, academic and social life of the affected individuals. Some individuals hide their hands under their arms, behind their back or under their thighs to avoid embracement.

Much of what makes us who we are is how others perceive us and importantly how we perceive ourselves. People with excessive sweating constantly struggle with this problem throughout their lives. Anxiety can accumulate when one fears embarrassment from excessive sweating or facial blushing. Ultimately one's career choices and relationships can suffer because of a condition one has little if any control over.

Treatment for Pedal hyperhidrosis or excessive foot sweating

Pedal hyperhidrosis or excessive foot sweating is usually accompanied by excessive hand sweating. This could be a part of a general excessive sweating syndrome consisting of a combination of hand, feet, face or armpit sweating.

Although foot sweating is not as obvious as hand sweating, it can still cause serious problems. Solitary foot sweating is relatively rare but foot sweating in combination with sweaty palms is more common. Specific complaints from the patients are:

    Slipping in shoes

    Odor problems

    Avoiding pedicures

    Infections and athletes foot

Excessive facial sweating another manifestation of hyperhidrosis.

Excessive facial sweating is another manifestation of hyperhidrosis. This can affect the face, scalp, and the neck. The hair can get a characteristic oily appearance, which could be unattractive.

Blow-drying wet hair becomes a problem. But perhaps the most troublesome problem associated with facial sweating is the social embarrassment.
Spontaneous facial sweating can start while eating, speaking to people or just being in a crowd. Women have trouble wearing makeup because it runs over their face. Facial sweating may occur alone or in combination with facial blushing.
Various medications such Robinul are effective in treating facial hyperhidrosis.Sympathectomy has also been used to treat excessive facial sweating alone or in combination with facial blushing.

Hyperhidrosis : Causes for excessive sweating

Excessive sweating or hyperhidrosis is defined as sweating beyond the physiological need of the body. Sweating is a way the body regulates its temperature. Sweat is produced by sweat glands located in the skin in various parts of the body.
These sweat glands have their highest concentrations in the palms of the hands. The activity of sweat glands is under the control of the sympathetic nervous system and is involuntary. Over activity of the sympathetic nervous system can cause excessive sweating.
Excessive sweating or hyperhidrosis is divided into two main categories:

    Primary hyperhidrosis:

Excessive sweating in primary hyperhidrosis has unknown etiology and tends to be localized. This can affect:
    Hands: palmar hyperhidrosis
    Face: facial hyperhidrosis
    Underarm: axillary hyperhidrosis
    Feet: pedal hyperhidrosis
Treatment for primary hyperhidrosis is a multidisciplinary challenge that involves medications, antiperspirants, iontophoresis and sympathectomy.

    Secondary hyperhidrosis:

Excessive sweating in secondary hyperhidrosis has an underlying reason and generally affects the entire body. Conditions causing secondary hyperhidrosis are:
    Hyperthyroidism
    Infections
    Malignancy
    Autoimmune disorders
    Obesity
    Menopause
    Diabetes
    Psychiatric disorders
Treatment for secondary hyperhidrosis involves eliminating the underlying condition.

Hyperhidrosis : Causes for excessive nighttime sweating.

Sweat glands in the hands are predominantly under the control of the sympathetic nervous system and are stimulated by emotional stimuli. Therefore patients with hyperhidrosis usually do not have excessive sweating during sleep or sedation. Thermal sweating however can occur during sleep and is one of the more important causes of nighttime sweating.
Nighttime sweating however may also be related to secondary hyperhidrosis with an underlying cause. In patients with secondary hyperhidrosis, causes for nighttime sweating can be recognized and treated after a complete diagnostic work up by a qualified physician.

Hyperhidrosis : Causes for excessive Perspiration.

Hyperhidrosis is excessive sweating. Hyperhidrosis is defined as the production of 100mg of sweat in the axilla over 5 minutes. The human body has 2-4 million eccrine sweat glands which function to cool the body by perspiration.

Innervations of these sweat glands are from the hypothalamic preoptic sweat center in the brain through brainstem and medulla. These nerves then enter the spinal cord and synapse in the anterolateral cell column of the spinal cord. These neurons subsequently exit the spinal cord and enter the sympathetic ganglia in the chest and finally supply the upper limbs.

It is thought that the over stimulation of the sweat glands in hyperhidrosis is related to the over activity of the sympathetic nervous system. Perspiration or sweating can be induced by emotional stress of thermal stimuli.
Emotional sweating is more common in the hands, underarm and feet. Subsequently emotional sweating or perspiration stops during sleep but thermal sweating can occur during sleep.

    TREATMENT:

    1. Information on Iontophoresis used in treating excessive hand sweating.

Iontophoresis works by inducing electrical changes in the sweat glands that disrupts the sweat production. This works by placing the affected areas in tap water. A small electric current is then passed through the skin.
This device works well for mild to moderate forms of hyperhidrosis. However its drawback is the time that is required to do the treatments. The treatments also have to be continued indefinitely. Treatment starts with twenty minutes on a daily basis and is tailored to individual symptoms
Side effects of iontophoresis are few. Affected areas may crack, become too dry or fissured. If this occurs the frequency of treatments should be decreased. Most patients report a tingling sensation and mild pain especially in the beginning of the treatments.

    2. ANTIPERSPIRANTS:

Antiperspirants work by decreasing sweat production from the eccrine and apocrine sweat glands. Unfortunately most patients with hyperhidrosis do not find commercial non-medical antiperspirants or deodorants useful enough. One exception to this is Aluminum Chloride in high concentration found in over the counter products.
Drysol is a solution of 20% Aluminum Chloride in anhydrous ethyl alcohol. Drysol is a prescription only medication, which is moderately effective in treating palmar and axillary hyperhidrosis. However it may cause severe skin irritation. Drysol is applied to the affected areas and left on for 6 hours. This is best done at nighttime before going to sleep. The medication is then completely washed off the next morning. Initial success rates are good but long-term results are less satisfactory.
Xertac AC is another medical antiperspirant used in treating hyperhidrosis, but it is not as effective as Drysol. Tannic acid in ethanol and formalin solutions has also been used to treat hyperhidrosis but they have been associated with allergic sensitization. Other useful topical antiperspirants are Odaban and Maxim.
In general topical antiperspirants have to be applied at nights and work best if used with saran wrap or plastic gloves for maximal effect.
