    Default Philhealth contribution

Hi guys! Need your suggestions. So I'm here on this company going to resign because I got a job on another company. I just found out that my contributions from May 2014 (startmonth) until now, wala pa na remit. Nagfloat ra daw. Didn't even notify us. HR said I have to go personally sa PhilHealth to apply and give them the form after. The catch is, my new company is asking my PhilHealth number. The question is, should I apply the PhilHealth through my original company or through my new company? My new work starts on Jan. 5, and I will have 30 days to pass the Philhealth number.
