True immortality is impossible and here's why
Let's say medical science has reached its highest point. There are no diseases that can't be cured. To fight the decay of aging. New organs can be grown and replace your old organs as you need them. Somehow, there are methods to keep your brain cells healthy. Let's also say you can access these medical services easily. And for the sake of the discussion, trauma related deaths like getting crushed, explosions, etc. are avoided.

Congrats! You're one of the oldest humans alive. Humanity has probably spread beyond earth to cope with population. But whatever it is, you'll go along with it

Livable planets are hard to find now. Most of them are now consumed by their parent starts. You might try to live on rogue planets but it's too cold and dark for your taste. It would probably be best if you and other survivors created your own artificial sun. You also need to research and discover new physics to stop the inevitable.

There is just you and black holes in the universe. How can you still be alive? Even the black holes are dying by Hawking radiation. You still live on...
Beyond
There are still a few particles left. Photons, Electrons floating in the dark abyss. They hardly ever interact with each other anymore. The "you" that was once was is gone. The particles that make up your body have decayed. Irrecoverable. Gone forever.
